CC-Hook

The CC-Hook is a single-patient-use handheld instrument with a curved end that is capable of passing under and around the coracoid process while causing minimal damage to the surrounding soft tissues.

INDICATIONS

It is cannulated and supplied with a flexible nitinol wire that is passed through the length of the instrument so that it protrudes from the far end of the device to capture a synthetic ligament or suture which can then be pulled back under the coracoid process. It is available in both Right and Left hand configurations. The Xiros CC Hook is specifically designed to facilitate this procedure.  Refer to the Instructions for Use (IFU) for specific instrument selection and usage.

 

HOW DOES IT WORK?

  1. With the patient in the beach chair position, make a 5 cm vertical incision starting at the clavicle, 1-2 cm medial to the tip of the coracoid. Ensure the incision is lateral to the planned button placement, approximately 3-4 cm from the distal tip of the coracoid. Incise the fascia and the deltoid vertically, then divide the periosteum over the posterior clavicle laterally, extending to the AC joint.

  2. Confirm that the CC-Hook is preloaded with the Nitinol wire or reload if necessary. To reload, take the first wire or uncoil the second Nitinol wire and pass the looped end into the back of the instrument handle. Push the wire forward, guiding it through the opening at the thumb grip until the loop is positioned just inside the tip of the hook.

  3. Expose the superior surface of the coracoid and carefully pass the hooked tip underneath and around it from medial to lateral. Follow the surface of the bone until the hooked tip visibly protrudes from the soft tissue on the lateral side.

  4. Push the wire forward either at the thumb grip or from the back of the handle until the loop extends from the hooked tip of the instrument.

  5. Pass the end of the implant through the protruding loop of the Nitinol wire. Secure it by retracting the wire until the implant is firmly trapped against the tip of the instrument. Hold the Nitinol wire at the thumb grip to prevent the implant from slipping.

  6. Carefully withdraw the instrument back around the coracoid process and out through the medial side, ensuring the implant follows the same path.

  7. Release the implant from the Nitinol wire loop and remove the instrument, completing the placement.
